Step 1: Gather your Supporting Documentation

- Before starting your application, make sure to gather supporting documentation that verifies your disability.
- Applications without documentation will be placed on hold, and medical forms completed by students will not be reviewed.

What counts as acceptable documentation?
- Educational or medical records, reports, or assessments created by healthcare providers, school psychologists, teachers, or the education system.
- The DASS Disability Verification Form (, 623KB)completed by a licensed or certified professional qualified to diagnose and treat your disability.

Important Notes:
- If you do not have the documentation listed above, you can complete a Self-Report Form (, 630KB). However, if your disability is not evident, DASS will request additional supporting documentation.
Step 2: Submit the
- If you did not submit your supporting documentation with your application, email it to accommodations@csuci.edu as soon as possible.
Step 3: Meet with a DASS Counselor to review your application.

DASS will contact you to schedule a meeting once your application is complete.
Note: Academic Accommodations and Housing Accommodations have two different application processes. For DASS Housing Accommodations, students are encouraged to apply by the deadline.
